2008 Football Registration Forms
To Save Time, Please Print And Fill Out The Forms Completely
The medical and registration forms needs to be completely filled out by the parent. Registration form page one (1) needs to be completed and page two (2) only needs the top section to be filled out and notarized.
For your convenience we will have a notary at registration.
Every child will also need a physical before they can participate. It needs to be dated after May 1st. of this year. You can use the section on the consent form for the physical or the doctor can just write on a script or his own form that the child may participate.
If this is the first year playing with us, we will need a copy of your child's birth certificate.

If you DO NOT live in Grosse Ile or live North of Harrison Road in Trenton or if live in any other city you are required to fill out the Standard Waiver form.
Please Note: All forms must be complete and physicals MUST be turned in no later than August 1, 2008. The Downriver Junior Football League manidates that ALL FORMS must be completed before anyone can practice.
